As part of your dog's ongoing Cushing's Syndrome treatment, families are asked to complete and submit the below questionnaire at least 24 hours before each review appointment. Please discuss the questions with everyone in your household and complete the questionnaire with these responses in mind.

We combine your home observations with laboratory test results and veterinary assessments to track your pet's progress.
